To make questions in the past tense we use:
did + subject + verb stem
Did he go to London?
Did they play football?
When did she leave?
How did he die?
Note: we use the verb stem, not the past form of the verb.
The past tense is the same for all persons.

A Read this information about Buddy Holly.
Charles Hardion Holly came from Lubbock, Texas. He was born on 7 September 1936. He made his first hit record in summer 1957. In February 1959 he died in a plane crash. In that short time he had seven hit singles and made two albums. But Holly wasn't just a singer. He was the first singer/songwriter in pop music. He wrote 33 songs. He played with a group called the Crickets. Their first hit record was 'That'll be the Day'. In February 1958 the Crickets went to Australia and then to Britain. In August 1958 Buddy Holly married Maria Elena Santiago. Later that year, Holly moved to New York. On 2 February 1959 he gave his last concert in Clear Lake, Iowa. That night, on 3 February, his plane crashed.
