В отрицании в past simple основной глагол стоит в настоящем времени 
2. The porter knew that the train ... be late.
a)would 
Прошедшее время, значит will не подходит. Had с инфинитивом to be не употребляется 
3. The waiter said that there ... no more milk.
a)was 
Прошедшее время + конструкция there is/was-are/were - is,are (настоящее время) не подходят, had совсем мимо.
4.Mr.Smith told me that I was ... very well.
b)speaking 
past continuous
was speak - абсурд, не бывает такого в природе :)
was spoken - страдательный залог; теоретически возможно, но по смыслу не совсем подходит и был бы соответствующий предлог.
