3 went: (a) succession of actions in the past: started playing then went to America OR (b) a definite point in the past: she was four when she started, three years later ( she was seven) she went to America
5 went: a definite point in the past May 15th

No, we have to use the Past Simple in those sentences, that's for sure

yes, you did. But we always use Present perfect if has preposition "to" or is there exclusion for it
