If the windows "are" cleaned, then you can't use "don't". You have to use "aren't". So instead of "The windows are cleaned once a month, don't they?", it's "The windows are cleaned once a month, aren't they?".
"What cleaned once a month?" is basically "Что мыть раз в месяц? ". So it's "What is (being) cleaned once a month?" - "Что моют раз в месяц?". This one's harder to explain in translation, but the latter is correct.
